speedy self-reporting system of your private
property vehicle crash. Please follow the
instructions below when completing the form.
DO NOT USE THIS FORM IF THE COLLISION
INVOLVES ANY
OF THE FOLLOWING:
(Dial 911 to have an
officer or deputy dispatched
to investigate if ANY apply)
- The driver of either vehicle is under
the influence of drugs or alcohol.
- The collision involves reckless driving.
- Personal injuries occurred.
- The collision occurred on a public
street.
- The license plate number of a
hit-and-run vehicle was obtained by a
witness.
INSTRUCTIONS FOR COMPLETION OF FORM
Step One: Complete all
requested information on the form as indicated
(see the example above).
Step Two: Print out necessary
copies for your records. You may need a
copy for your insurance company.
Step Three: E-mail the form to
Hillsdale County Central Dispatch. You may
also print and send a copy of the report form to
the appropriate agency indicated below.
